So is Castiel, Sam?! That was actually my first thought, but if it were so that wouldn't change anything. If Dean is already born, and Sam isn't I guess Cas could be a middle child? There is four years between Sam and Dean. Cas definately couldn't be born after Sam, there wouldn't be time.
