Oh, the drive to the house was just ... edge of the seat, really, even though we'd already been assured by the opening line that they made it there. But all the Dean!pain was just *flail* and the Sam POV with the worry and the love and all that - and then John telling him to keep Dean awake even if he had to use *more* pain to do it. (And I couldn't help but think "oh no, their new van is all stained with demon and Dean blood now; that just won't ever come out", a reaction that I think means I'm getting old.)
It's all the little details that really sell the story. Dean's eyes moving behind closed lids, his nails digging into the arm of the van seat ... and more stuff - too much to quote - throughout the story. And that demon-driven fever! Eeek!
The medical routine felt right. Triage assessment, Rafe letting the other two put him off while he attended Dean first (corset! *giggle*), but then insisting on checking them too. John's problem with needles! Sam's teasing banter (containing, can't help but think, utter relief that they've got the time for teasing now) about John's problem with needles and John's quick rebuttals.
And *ick* wound care. And John's poor mistreated stump! (I spent quite a time in X-Files fandom being a huge Krycek fangirl, so the whole taking care of the amputation site idea is just *ground* into me from that. And also, of course, Xander's eye, though that's kinda a different kind of wound/loss.) And Rafe's frustrated acceptance that John *wouldn't* take proper care of it.
And it's just so Dean to fight the sedation and need an extra wallop to get put under. Let go for a minute, boy, your daddy and brother are there for you now, you can let go!

And just as things start to really relax, Dean's sedated, John and Sam have had medical attention, too -- and *whump* that last line, putting it all back on the edge. *eeep*
