Amazing take on Dean's psyche. He's still trying to do the right thing on principle. It says a lot about his strength of character. And what's tragic is that the very things he fears- Sam's death, John not loving him enough- are not even true except in his mind. You can feel him disintegrating but trying so hard to hold on. Like everyone else, I'm anxiously awaiting the family reunion. Just hope the troops get there in time to save him!
